Tinjauan Naratif Risiko dan Manfaat Penggunaan Kortikosteroid Sistemik pada Pasien PPOK dengan Pneumonia

Abstract

Systemic corticosteroids are a standard treatment for acute exacerbations of Chronic Obstructive Pulmonary Disease (COPD). However, their use in COPD patients with concomitant pneumonia remains controversial because the balance between benefits and risks is unclear. This narrative review evaluated the benefits and risks of systemic corticosteroid therapy in COPD patients with pneumonia. Relevant articles published between 2017 and 2025 were identified through PubMed, Scopus, ScienceDirect, and Google Scholar. Eligible studies were analyzed and synthesized narratively to assess clinical outcomes. Systemic corticosteroids may provide benefits by reducing inflammation and improving respiratory symptoms. Nevertheless, most studies reported no significant improvement in mortality, treatment failure, intensive care unit admission, or rehospitalization rates. Conversely, corticosteroid use was associated with prolonged hospital stay, potential drug interactions, and immunosuppressive effects that may delay infection resolution. Overall, the available evidence suggests that the clinical benefits of systemic corticosteroids in COPD patients with concomitant pneumonia remain uncertain and should be weighed carefully against potential risks. Therefore, routine use is not recommended, and treatment decisions should be individualized according to the patient’s clinical condition.
